Each member of the Cornerstone Family Practice of Lexington staff looks forward to providing you with comprehensive care in a friendly, comfortable environment.

Family Medical Care

Family medicine provides comprehensive medical services to all members of the family. Dr. Thomas-Slade is board certified by the American Board of Family Practice and cares for your pediatric, adolescent and adult medical concerns including:

• Wellness health exams (including school and employment physicals)
• Sick visits for cold, flu and infections
• Newborn and infant care
• Gynecological examinations
• Minor office surgery (including repair of lacerations, toe nails, sprains and minor fractures, and treatment of skin lesions and skin abscesses)
• Treatment of most acute and chronic illnesses
• Ongoing management of chronic diseases (including diabetes and hypertension)
• Screenings for cancer, cholesterol, blood pressure, blood sugar and heart disease
• Onsite laboratory
• Workmen's Compensation

Other services we provide in our office include basic blood work and urine tests, and we can perform many other tests to assist with the diagnosis and care of our patients, including:

• Hearing tests
• Vision tests
• Breathing tests

Appointments

When you call for an appointment, please explain the nature of your problem to allow us to schedule it promptly and efficiently. If you have an urgent problem but do not have an appointment, please call our office so our triage nurse can make sure you are seen the same day when necessary. Please advise us if you will be late or unable to attend your appointment. If you must cancel, we request at least 24 hours notice. Patients who miss more than two appointments without such notice may be asked to leave our practice.

Patient Services

It is our policy to schedule an appointment to see Dr. Thomas-Slade for your medical problem, rather than diagnosing or prescribing for your condition over the phone. We believe that accurate diagnosis and treatment requires a physical examination and appropriate diagnostic testing.

Our staff is trained to answer many of your questions regarding medical care, and they consult Dr. Thomas-Slade regarding your call. If you need a prescription refill, please contact your pharmacy so they can call our office. This process allows your doctor and the pharmacy to review and complete your request more efficiently. Prescription refills, scheduling of routine appointments and other non-urgent questions are handled each weekday between the hours of 8:00 a.m. and 5:00 p.m.

Your Electronic Medical Record

All Cornerstone practices are connected to each other through a company-wide electronic medical record. Our patients receive many advantages through this paperless records system. Dr. Thomas-Slade has quick access to your medical record and is able to share your record with any Cornerstone specialist. She can determine which drugs are on your drug formulary and has access to formulary alternatives. She uses the information in the electronic medical record to work with you to create a customized preventive health plan. This health plan will give you target dates to return for ongoing health checkups. Patient confidentiality is carefully maintained with the utmost respect for your privacy. Information regarding your medical care will not be released to anyone without signed consent.

Referrals

If a member of your family needs a referral to other specialists or to community resources for specialized evaluation or treatment, we can help you select the resource most appropriate for your needs, and we will continue to work with you and your family to maintain continuity of care.

Fees and Insurance

At the completion of your visit, you will be advised of the fees for the day and any prior balance. The cost of your visit is determined by the time we spend with you, the complexity of your condition, and by the lab tests necessary to properly diagnose and care for you. In an effort to keep our fees fair and reasonable, we request payment of the portion of the bill that is not covered by your health insurance plan on the date of your visit with our office.

Our office is a participating provider with most major health insurance plans. Our office staff will be able to tell you if your plan is accepted by our practice. As a courtesy to our patients, we will file claims to your insurance company and provide them information to determine benefits. It is the responsibility of you and your insurance company to pay your medical bills. If your primary insurance is Medicare, please be aware that some secondary insurance policies do not cover your deductible. We suggest that you review your insurance policies carefully to determine what benefits are covered.
